About me
I become involved in patient safety following the loss of my baby son Joshua due to failures in his care at Morecambe Bay Trust in 2008. Working with other families, I successfully campaigned for the Morecambe Bay Investigation which was published in March 2015.
I have worked as the National Advisor on Safety for the Care Quality Commission and was a member of the expert advisory group established to set up the new Healthcare Safety Investigation Branch (HSIB). I regularly speak at national conferences on the subject of patient safety and have had articles published in journals such as the British Medical Journal, Health Service Journal and Nursing Times, as well as the national press.
